BACKGROUND: Dyspnoea is a common symptom of respiratory disease. However, data on its prevalence in general populations and its association with lung function are limited and are mainly from high-income countries. The aims of this study were to estimate the prevalence of dyspnoea across several world regions, and to investigate the association of dyspnoea with lung function. METHODS: Dyspnoea was assessed, and lung function measured in 25,806 adult participants of the multinational Burden of Obstructive Lung Disease study. Dyspnoea was defined as ≥2 on the modified Medical Research Council (mMRC) dyspnoea scale. The prevalence of dyspnoea was estimated for each of the study sites and compared across countries and world regions. Multivariable logistic regression was used to assess the association of dyspnoea with lung function in each site. Results were then pooled using random-effects meta-analysis. RESULTS: The prevalence of dyspnoea varied widely across sites without a clear geographical pattern. The mean prevalence of dyspnoea was 13.7 % (SD=8.2 %), ranging from 0 % in Mysore (India) to 28.8 % in Nampicuan-Talugtug (Philippines). Diabetic-metabolic syndrome (MetS-D) has a high prevalence worldwide, in which an association with the rupture of the intestinal epithelium barrier function (IEBF) has been pointed out, but the functional and morphological properties are still not well understood. This study aimed to evaluate the impact of acute hyperglycemia diabetes on intestinal tight junction proteins, metabolic failure, intestinal ion and water transports, and IEBF parameters. Diabetes was induced in male Rattus norvegicus (200-310 g) with 0.5 mL of streptozotocin (70 mg/kg). Glycemic and clinical parameters were evaluated every 7 days, and intestinal parameters were evaluated on the 14th day. The MetS-D animals showed a clinical pattern of hyperglycemia, with increases in the area of villi and crypts, lactulose:mannitol ratio, myeloperoxidase (MPO) activity, and intestinal tissue concentrations of malondialdehyde (MDA), but showed a reduction in reduced glutathione (GSH) when these parameters were compared to the control. The MetS-D group had increased secretion of Na+, K+, Cl-, and water compared to the control group in ileal tissue. Furthermore, we observed a reduction in mRNA transcript of claudin-2, claudin-15, and NHE3 and increases of SGLT-1 and ZO-1 in the MetS-D group. These results showed that MetS-D triggered intestinal tissue inflammation, oxidative stress, complex alterations in gene regulatory protein transcriptions of intestinal transporters and tight junctions, damaging the IEBF and causing hydroelectrolyte secretion.

The rubber tree (Hevea brasiliensis (Willd. ex A.Juss.) Müll.Arg.), a native Amazonian species, is responsible for more than 50 thousand products and stands out as the world's main source of natural rubber. Commercial production is carried out by grafting, however, the technique has gaps in terms of time and quality for seedling production. Vegetative production via cuttings is an alternative, however, the species is difficult to root. Thus, the study tested the rooting induction, through a chemical method, with the hormonal regulator indolbutyric acid (IBA) of 5000 ppm, and a mechanical method, with the strangulation of stems, and the interaction between the methods, to analyze the survival and sprouting of rubber tree (Hevea brasiliensis) cuttings, as well as verifying the efficiency of breaking the sclerenchyma ring by strangulation. A randomized block design was used, with four treatments (control, with strangulation, with IBA, strangulation x IBA) distributed in six blocks with 36 cuttings. Data were submitted to ANOVA test and Tukey's post-test (p>0.05). The results obtained 12.5% of live cuttings, without rooting, during 68 days, being the combination of strangulation and IBA with greater survival and sprouting. No breakage of the sclerenchyma ring was observed by histological analysis. The data indicate strategic gains in combining chemical and mechanical techniques for species of difficult rooting in vegetative propagation, however, the test was not enough to affirm an answer in relation to each technique, the deepening of the technique on the behavior of the species remains the biggest challenge.

There were three purposes to this research. First, we sought to test the differential role of harmonious and obsessive passion in predicting intention and habit for exercise. The second goal of this research was to test the associations between intention, habit and exercise frequency. Last, the third goal was to assess the mediation role of intention and habit in the relationship between both forms of passion and exercise frequency. The sample consisted of 284 participants (140 males, 144 females) aged 18-60 years old (M age = 29.81, SD = 9.16). Regression paths of the structural model indicated significant associations: a) harmonious passion was significantly associated with intentions and habit; b) obsessive passion was significantly associated with habit; c) intentions and habit were significantly associated with exercise frequency. The mediation model variance explained was 17% (p < .001). The total effect via harmonious and obsessive passion was ß = .57 (IC95% = .42, .73) and the total indirect effect was ß = .11 (IC95% = .02, .24). Having harmonious passion for a fitness activity appears to have the potential to increase regular physical activity among exercisers. Individuals who recognize the significance of physical exercise and participate in activities they enjoy the most can affirm the positive effects on their health from being active.

Pre-emergent herbicides can contribute to the control of weed competition in direct seeding restoration, however it is necessary to evaluate their effects on seeds of native tropical forest species. The aim of the study was to assess the potential impact of the herbicide indaziflam on the germination of 17 forest species. For this, a dosage of 180 mL of the product in 200L of water was compared to the control without herbicide. The degree of sensitivity of each species was calculated by a ratio between the percentage of germination with herbicide (GH) and the control without herbicide (GC) classifying them as: extremely sensitive (ES= (GH/GC) <0.25), sensitive (S=0.25< (GH/GC) <0.50), low sensitivity (LS=0.50< (GH/GC) <0.75), indifferent (I=0.75< (GH/GC) <1.0) and potentiated (P= (GH/GC) >1). The herbicide promoted a significant reduction in mean germination in 35% (n=6) of the species and 59% (n = 10) were sensitive or extremely sensitive to indaziflam, and only three did not germinate. On the other hand, 29.4% (n=5) showed low sensitivity or indifference to the herbicide, while seed germination was slightly increased by indaziflam to 11.7% (n=2). Pre-emergent indaziflam can be recommended in direct seeding restoration, as only 17.6% (n=3) of the species were inhibited by pre-emergent. However, the effect of indaziflam varies by species and requires further studies to support large-scale use in direct seeding.

Atypical dengue prevalence was observed in 2020 in many dengue-endemic countries, including Brazil. Evidence suggests that the pandemic disrupted not only dengue dynamics due to changes in mobility patterns, but also several aspects of dengue surveillance, such as care seeking behavior, care availability, and monitoring systems. However, we lack a clear understanding of the overall impact on dengue in different parts of the country as well as the role of individual causal drivers. In this study, we estimated the gap between expected and observed dengue cases in 2020 using an interrupted time series design with forecasts from a neural network and a structural Bayesian time series model. We also decomposed the gap into the impacts of climate conditions, pandemic-induced changes in reporting, human susceptibility, and human mobility. We find that there is considerable variation across the country in both overall pandemic impact on dengue and the relative importance of individual drivers. Increased understanding of the causal mechanisms driving the 2020 dengue season helps mitigate some of the data gaps caused by the COVID-19 pandemic and is critical to developing effective public health interventions to control dengue in the future.

INTRODUCTION AND OBJECTIVES: Portugal is one of the countries with the highest number of visits to the emergency department (ED), 31% classified as "non-urgent" or "avoidable." The objectives of our study were to evaluate the size and characteristics of patients with pulmonary disease who overuse the ED, and identify factors associated with mortality. MATERIALS AND METHODS: A retrospective cohort study was conducted, based on the medical records of ED frequent users (ED-FU) with pulmonary disease who attended a university hospital center in the northern inner city of Lisbon from January 1 to December 31, 2019. To evaluate mortality, a follow-up until December 31, 2020 was performed. RESULTS: Over 5,567 (4.3%) patients were identified as ED-FU and 174 (0.14%) had pulmonary disease as the main clinical condition, accounting for 1,030 ED visits. 77.2% of ED visits were categorized as "urgent/very urgent." A high mean age (67.8 years), male gender, social and economic vulnerability, high burden of chronic disease and comorbidities, with a high degree of dependency, characterized the profile of these patients. A high proportion (33.9%) of patients did not have a family physician assigned and this was the most important factor associated with mortality (p<0.001; OR: 24.394; CI 95%: 6.777-87.805). Advanced cancer disease and autonomy deficit were other clinical factors that most determined the prognosis. CONCLUSIONS: Pulmonary ED-FU are a small group of ED-FU who constitute an aged and heterogeneous group with a high burden of chronic disease and disability. The lack of an assigned family physician was the most important factor associated with mortality, as well as advanced cancer disease and autonomy deficit.

Introducción y objetivos: La artroplastia total de cadera (THA) es un procedimiento traumatológico cada vez más común, que comporta dolor postoperatorio de moderado a severo. El bloqueo del grupo de nervios pericapsulares (PENG) es un bloqueo reciente que parece aportar analgesia adecuada sin bloqueo motor significativo. El objeto de este estudio es comparar retrospectivamente la eficacia analgésica y la seguridad del bloqueo PENG y la analgesia epidural, en pacientes sometidos a THA. Material y métodos: Se trata de un estudio retrospectivo observacional durante un periodo de un año de los pacientes sometidos a THA primaria, utilizando analgesia epidural o bloqueo PENG ecoguiado con inyección única. Los datos relativos a las características demográficas, técnicas quirúrgicas y anestésicas, puntuaciones de dolor, consumo de opioides, complicaciones y tiempo hasta el alta hospitalaria se recopilaron de los registros institucionales, comparándose entre los dos grupos (analgesia epidural vs. bloqueo PENG). Resultados: No se encontraron diferencias significativas en cuanto a las puntuaciones de dolor, el consumo de opioides y el tiempo medio hasta el alta hospitalaria entre los dos grupos. Las puntuaciones de dolor en reposo (1,2 epidural vs. 1,67 PENG) o con movimiento (3,95 epidural vs. 3,72 PENG) fueron similares entre ambos grupos. El número total de complicaciones fue superior en el grupo de analgesia epidural (50% epidural vs. 5% PENG). Se reportó parestesia en ambos grupos. Solo se reportaron bloqueo motor, sedación, náuseas y complicaciones relacionadas con el catéter en el grupo de analgesia epidural. Conclusiones: El bloqueo PENG parece ser equivalente a la analgesia epidural en cuanto a calidad de la analgesia postoperatoria para los pacientes sometidos a THA primaria, lo cual respalda el uso rutinario de este bloqueo en estos pacientes. La baja tasa de complicaciones reportadas limita las conclusiones sobre esta cuestión.(AU)

The present study explored the mediation role of past exercise adherence, self-reported frequency and intentions in the association between past experience and future exercise adherence. In total, 431 exercisers (female = 216; male = 215) aged 18 and 64 years, engaged in fitness activities such as group fitness classes and resistance training, were included in the analysis. Serial mediation procedures were employed to examine the direct, indirect, and total indirect effects among variables. The predictor variable and all mediators displayed a positive and significant association with future six-month adherence. Past six-month exercise adherence displayed the most significant association with future six-month adherence. The sequential indirect path from exercise experience → past six-months adherence → self-reported frequency → intentions future six-months adherence displayed a positive and significant effect (ß = .19 [CI95% = .09, .31]), presenting a partial mediation effect. Past behaviour is the most significant predictor of future adherence, and thus interventions should be based on promoting consistent exercise frequency. Professionals working in the fitness centre context can identify possible dropouts based on their past behaviour and intentions to be physically active in the future.

El concepto de pasión se refiere a una fuerte inclinación a realizar una actividad que las personas disfrutan, que encuentran importante y en la que invierten tiempo y energía. El Modelo Dualístico de la Pasión explora este concepto en las dimensiones armoniosa y obsesiva y sus implicaciones en la motivación y parámetros asociados a la realización de conductas. Para apoyar la modificación conductual orientada a la práctica del ejercicio, son cruciales instrumentos adecuados y de calidad para evaluar la pasión. El objetivo principal del presente estudio fue analizar la validez y confiabilidad de la Escala de la Pasión en practicantes de ejercicio físico en el contexto del gimnasio y clubes de salud. La muestra estuvo compuesta por 203 participantes (sexo femenino = 109; sexo masculino = 94), con edades comprendidas entre 18 y 68 (M = 29.24; DT = 9.76) años. Los resultados de este estudio indican que el modelo de medición original de la Escala de Pasión de 16 ítems presentó un ajuste aceptable a los datos. Con la excepción de la pasión obsesiva por el hábito, todos los factores de la escala mostraron asociaciones positivas con la intención y el hábito. Los datos de este estudio sugieren que el modelo de medición Passion Scale (16 ítems/tres factores) adaptado al contexto del ejercicio se ajusta a los datos de la muestra del estudio y es una herramienta viable para el estudio de los factores involucrados en la adherencia continua en la práctica. The concept of passion refers to a strong inclination to carry out an activity that people enjoy, that they find important and in which they invest time and energy. The Dualistic Model of Passion explores this concept in the harmonious and obsessive dimensions and its implications for motivation and parameters associated with the performance of behaviors. To support behavioral modification oriented towards the practice of exercise, adequateand quality instruments to assess passion are crucial.The main objective of the present study was to analyze the validity and reliability of the Passion Scale in practitioners of physical exercise in the context of the gym and health clubs. The sample consisted of 203 participants (female sex = 109; male sex = 94), aged between 18 and 68 (M = 29.24; SD = 9.76) years.The results of this study indicate that the original 16-item Passion Scale measurement model presented an acceptable fit to the data. Exceptfor the association between obsessive passion on habit, all factors on the scale showed positive associations with intention and habit. The data from this study suggest that the Passion Scale measurement model (16 items/three factors) adapted to the exercise context fits the data in the study sample and is a viable tool for the study of factors involved in continued adherence in practice. O conceito de paixão refere-se a uma forte inclinação para a realização de uma atividade que as pessoas gostam, que acham importante e na qual investem tempo e energia. O Modelo Dualístico da Paixão explora esse conceito nas dimensões harmoniosa e obsessiva e suas implicações na motivação e parâmetros associados à realização de comportamentos. Visando o apoio à modificação comportamental orientada para a prática de exercício, instrumentos adequados e com qualidade para avaliar a paixão são determinantes. O objetivo do presente estudo foi analisar a validade e a fiabilidade da Passion Scale em praticantes de exercício físico no contexto do ginásio e health clubs. A amostra foi constituída por 203 participantes (sexo feminino = 109; sexo masculino = 94), com idades compreendidas entre 18 e 68 (M = 29.24; DP = 9.76) anos. Os resultados deste estudo indicam que o modelo de medida original de 16 itens da Passion Scale apresentou um ajustamento aceitável aos dados. Há exceção da paixão obsessiva com o hábito, todos os fatores da escala apresentaram associações positivas com a intenção e hábito. Os dados deste estudo sugerem que o modelo de medida da Passion Scale (16 itens/três fatores) adaptado ao contexto do exercício se ajusta aos dados na amostra em estudo e é uma ferramenta viável para o estudo de fatores envolvidos à adesão continuada na prática. The mainly aim of this study was to use mucilaginous solutions obtained from tamarind, mutamba, cassia tora, psyllium and konjac powdered to encapsulate reuterin-producing Limosilactobacillus reuteri in alginate beads by extrusion technique. In the particles were determined the bacterial encapsulation efficiency, cell viability during storage and survival under simulated gastric and intestinal conditions. Moreover, the reuterin production, its entrapment into the beads and the influence on viability of encapsulated microorganism were evaluated. Scanning electron microscopy and Fourier Transform Infrared spectroscopy were employed to characterize the produced particles. The beads showed a relatively spherical shape with homogenous distribution of L. reuteri. The use of gums and mucilages combined with alginate improved the encapsulation efficiency (from 93.2 to 97.4%), the viability of encapsulated bacteria during refrigerated storage (especially in prolonged storage of 20, 30 and 60 days) and the survival after exposure to gastric and enteric environments (from 67.7 to 76.6%). The L. reuteri was able to produce reuterin via bioconversion of glycerol in the film-forming solutions, and the entrapment of the metabolite was improved using konjac, mutamba and tamarind mucilaginous solutions in the encapsulation process (45, 44.57 and 41.25%, respectively). Thus, our findings confirm the great potential of these hydrocolloids to different further purposes, enabling its application as support material for delivery of chemical or biological compounds.

This article presents a comprehensive overview of multicomponent reactions (MCRs) that proceed via ortho-quinone methide intermediates (o-QM) generated in the reaction medium. Examples of applications involving these highly reactive intermediates in organic synthesis and biological processes (e. g., biosynthetic pathways, prodrug cleavage and electrophilic capture of biological nucleophiles) are also described. QMs are often generated by eliminative processes of phenol derivatives or by photochemical reactions, including reversible generation in photochromic substances. This class of compounds can undergo various reaction types, including nucleophilic attack at the methide carbon, with subsequent rearomatization, and react with electron-rich dienophiles in inverse-electron demand hetero-Diels-Alder reactions. Its versatile reactivity has been explored in the context of cascade reactions for the construction of several classes of substances, including complex natural products.

O presente estudo analisou a associação entre estados humor e Síndrome de Overtraining (SOT) em jovens atletas e as possíveis diferenças nos estados de humor e na SOT em função do nível competitivo e em função das idades. Métodos: No total, 219 jovens atletas, com idades compreendidas entre os 13 e os 17 anos (M = 14.81; DP = 1.35), foram considerados para análise deste estudo. Resultados: Os resultados demonstraram que existe uma relação positiva e significativa entre a depressão, o desajuste ao treino, a confusão (negativo) e a tensão com a SOT. Foram, também, encontradas diferenças significativas nos estados de humor hostilidade e depressão em função do nível competitivo e nos estados de humor confusão (positivo) e hostilidade em função da idade. Conclusão Estes dados indicam, não só, que existe uma associação positiva entre estados de humor negativos e a SOT, como a existência de diferenças entre níveis competitivos e entre idades, sendo, por isso, necessário ter em conta essas variáveis. Este estudio analizó la asociación entre los estados de ánimo y el Síndrome de Overtraining (SOT) en deportistas jóvenes y las posibles diferencias en los estados de ánimo y en SOT considerando el nivel competitivo y los grupos de edad. Métodos: En total, 219 atletas jóvenes, con edades entre 13 y 17 años (M = 14.81; DT = 1.35), fueron considerados para el análisis en este estudio. Resultados: Los resultados demuestran que existe una relación positiva y significativa entre depresión, inadaptación al entrenamiento, confusión (negativa) y tensión con SOT. También se encontraron diferencias significativas en los estados de ánimo, ira y depresión a nivel competitivo, y en confusión (positiva) e ira según la edad. Conclusiones: Estos datos indican, no solo, que existe una asociación positiva entre estados de ánimo negativos y SOT, así como la existencia de diferencias entre niveles competitivos y entre grupos de edad, por lo que es necesario tener en cuenta estas variables. This study analyzed the association between mood states and Overtraining Syndrome (OTS) in young athletes and the possible differences in mood states and in OTS considering the competitive level and age groups. Methods: In total, 219 young athletes, aged between 13 and 17 years old (M = 14.81; SD = 1.35), were considered for analysis in this study. Results: The results demonstrate that there is a positive and significant relationship between depression, maladjustment to training, confusion (negative) and tension with OTS. Significant differences were also found in the mood states, anger and depression at the competitive level, and in confusion (positive) and anger according to age. Conclusions: These data indicate, not only, that there is a positive association between negative mood states and OTS, as well as the existence of differences between competitive levels and between age groups, therefore, it is necessary to take these variables into account. Purpose: To date, most research on the assessment of motivation has been exerciser-focused and has not considered how fitness professionals' motivations impact their behaviors toward exercisers during training sessions. The purpose of this study was to examine the factor structure of the Coach Motivation Questionnaire in a sample of fitness professionals (CMQ-FP) to ascertain its usefulness for this vocational grouping. Measurement invariance analysis was conducted between female and male fitness professionals, and predictive validity was tested considering need-supportive and need-thwarting behaviors as outcomes. Methods: Participants were 799 fitness professionals (female = 412) aged between 20 and 56 years (M = 28.71, SD = 3.24), who completed a multi-section survey assessing their motivation toward work and their interpersonal behaviors when engaging with exercisers. Results: The results of this research supported all three hypotheses. First, the hypothesized 6-factor measurement model showed acceptable fit to the data. Second, the factor structure of the CMQ-FP was invariant across gender (male and female fitness professionals). Third, fitness professionals' (autonomous or controlled) motivation was a valid predictor of need-supportive or need-thwarting behaviors. Conclusion: This study supported the factor structure of the CMQ-FP, presenting as a valid measure of motivation in fitness professionals. Understanding fitness professionals' perceptions of their coaching motivation can inform professional development activities to assist fitness professionals to increase understanding of what motivates these professionals and how they might be more need-supportive and less need-thwarting in their pedagogical behaviors.

Limosilactobacillus reuteri produces reuterin via glycerol anaerobic fermentation. This compound has antimicrobial properties and is used for food preservation purposes. Filmogenic solutions constituted of polysaccharides and glycerol are also employed, however, reuterin synthesis in filmogenic solutions has not yet been reported. Thus, the aim of this study was to optimize the in situ reuterin production by L. reuteri in alginate- and glycerol based-filmogenic solution, evaluating the survival of reuterin-producing bacteria during fermentation. The study consisted of a completely randomized design employing two L. reuteri strains (DSM 20016 and DSM 17938). The filmogenic solutions were obtained using sodium alginate (20 g/L) and two independent variables were studied: glycerol (0-300 mmol/L) and initial biomass of L. reuteri (≅6, 7, and 8 log CFU/mL). The samples were analyzed every 24 h for 72 h of anaerobic fermentation (37 °C). Both L. reuteri strains confirmed the potential for reuterin production and were susceptible to the metabolite produced. The highest reuterin production was achieved using L. reuteri DSM 20016. The initial microbial biomass of 8 log CFU/mL and 100 mmol/L of glycerol increased the reuterin production. However, higher conversion yields from glycerol to reuterin were obtained using 50 mmol/L of substrate.

Cauda equina syndrome (CES) is a rare but possible complication of neuroaxial anesthesia. Damage to the nerve roots may occur due to compression, inflammation, stretching, direct trauma, spinal ischemia or neurotoxicity, usually with lidocaine or bupivacaine. We describe a case of a 33-year-old patient that underwent an uneventful cesarean section with a combined spinal-epidural technique anesthesia, with levobupivacaine. 48 h after the procedure, she presented diminished muscular strength and abolished osteotendinous reflexes in the left lower limb, limited flexion of the right hallux, urinary retention and saddle anesthesia. Imaging exams excluded hematoma, thickening or compression of the cauda equina nerve roots. CES was suspected and treatment was initiated. 9-month follow up revealed diminished osteotendinous reflexes on the left lower limb and perianal hypoesthesia. Despite being unusual, neurological complications require prompt recognition and management to avoid permanent damage.
